Free Bipolar Screening Test (MDQ)

The Mood Disorder Questionnaire screens for periods of elevated, irritable or unusually energetic mood β€” the part of bipolar disorder that people rarely think to mention.

These screenings are for awareness only and are not a diagnosis. If you are in crisis or thinking of harming yourself, please call your local emergency number or a mental-health helpline immediately, or visit the nearest hospital.

About this test

Bipolar disorder is very often diagnosed late, because people seek help when they are depressed and nobody asks about the other half of the picture. The MDQ asks specifically about periods when you were "not your usual self" β€” unusually confident, needing little sleep, talking faster, taking risks.

Scoring has three parts, and all three matter: at least seven of the thirteen symptoms, several of them happening in the same period, and that period causing real problems. Only then is it a positive screen.

This distinction matters clinically, because bipolar depression is not always treated the same way as unipolar depression. Getting the picture right changes the treatment.

Frequently asked questions

Does a positive screen mean I have bipolar disorder?
No. The MDQ is a screening questionnaire; it identifies people for whom a proper psychiatric assessment is worthwhile. Many who screen positive do not have bipolar disorder.
I have been treated for depression for years. Why does this matter?
Because if episodes of elevated mood have been part of the picture, the diagnosis β€” and the right medication β€” may be different. It is a common and important thing to check.
What if I only had one brief period like that?
Mention it anyway. Duration and severity are exactly what a clinician needs to weigh.
Is bipolar disorder treatable?
Yes. With the right medication and support, most people with bipolar disorder live stable, full lives.
